Pesticide Application for Mosquito Control by Orient Mosquito District from May 1, 2011, to October 31, 2011
The Orient Mosquito District will conduct a mosquito control program in the Village of Orient from May 1, 2011, to October 31, 2011.
I. PESTICIDES USED FOR MOSQUITO CONTROL:
The following pesticides are used by the Orient Mosquito District in its mosquito control program. These pesticides are registered by the United Stated Environmental Protection Agency (EPA) and the New York State Department of Environmental Conservation (DEC).
Larvicides (Ground Application)
ALTOSID BRIQUETS (EPA REG. NO. 2724-375 8.62% METHOPRENE). The EPA Toxicity Category Signal Word for this pesticide is CAUTION. Keep out of reach of children. Causes moderate eye irritation. Harmful if absorbed through skin. Avoid contact with skin, eyes or clothing. Wash thoroughly with soap and water after handling and before eating, drinking, chewing gum, or using tobacco. Remove and wash contaminated clothing before reuse. Do not apply to known fish habitats. Do not contaminate water when disposing of unused product.
VECTOBAC-G (EPA Reg. No. 73049-10) 0.92% BTI. The EPA Toxicity Category Signal Word for this pesticide is CAUTION. Keep out of reach of children. Causes moderate eye irritation. Avoid contact with skin, eyes or clothing. Wash thoroughly with soap and water after handling. Do not contaminate water when cleaning equipment or disposing of equipment washwaters. Do not apply directly to treated, finished drinking water reservoirs or drinking water receptacles when the water is intended for human consumption.
VECTOLEX-CG (EPA Reg. No. 73049-20) 7.5% Bacillus sphaericus. The EPA Toxicity Category Signal Word for this pesticide is CAUTION. Keep out of reach of children. Harmful if absorbed through skin. Causes moderate eye irritation. Avoid contact with skin, eyes or clothing. Wash thoroughly with soap and water after handling. Do not contaminate water when disposing of equipment washwaters or rinsate. Do not apply directly to treated, finished drinking water reservoirs or drinking water receptacles when the water is intended for human consumption.
Adulticides (Ground Application)
SCOURGE (EPA Reg. No. 432-716) 4.14% Resmethrin-The EPA Toxicity Category Signal Word for this pesticide is CAUTION. Keep out of reach of children. Harmful if swallowed or absorbed through skin. Avoid contact with skin, eyes or clothing. Wash thoroughly with soap and water after handling. This pesticide is highly toxic to fish. For terrestrial uses, do not apply directly to water, to areas where surface water is present or to intertidal areas below the mean high water mark. Drift and runoff from treated sites may be hazardous to fish in adjacent waters.
II. SYMPTOMS OF PESTICIDE POISONING INCLUDE: Blurred vision; Dizziness; Vomiting; Headaches; Itching or Rash; Irritation of Eyes, Skin or throat; Chest Pains; or Excessive Sweating.
III. PESTICIDE APPLICATION WILL BE MADE TO THE FOLLOWING AREAS ON THE SCHEDULE PROVIDED BELOW.
WEEKLY GROUND PESTICIDE APPLICATIONS FOR LARVAL CONTROL:
Will be applied by ground application once a week, if needed, to mosquito sites in approximately 600 salt marsh acres bordering Gardiners Bay in the Village of Orient from Peters Neck to Orient State Park and north to the Main Road as well as fresh water ponds and catch basins in the Orient Mosquito District.
WEEKLY GROUND APPLICATION FOR ADULT MOSQUITO CONTROL:
SCOURGE will be applied by ground application equipment (portable or truck mounted ultra low equipment) throughout ORIENT from May 1 to October 31 as requested or as required.
IV. BEEKEEPERS SHOULD NOTIFY THE ORIENT MOSQUITO DISTRICT AT THE ADDRESS BELOW OF THE LOCATION OF ALL OF THEIR HIVES.
V. FURTHER PESTICIDE AND APPLICATION INFORMATION IS AVAILABLE FROM ORIENT MOSQUITO DISTRICT.

Copies of said documents may be obtained from Tetra Tech Architects & Engineers at the above address, by Bidders for Prime Contract Work upon payment of a deposit of $ 100.00 for each complete set. All checks for sets of Bidding and Contract Documents, complete or partial, shall be made payable to the Greenport Union Free School District.
Partial sets or sections of the Contract Documents for use by subcontractors may be obtained from the Architect upon making a written request listing the drawing numbers and Project Manual sections desired, and upon payment equal to the cost of duplicating same; no part of such payment shall be refunded. Subcontractors obtaining partial sets or sections of the Contract Documents are advised that they remain responsible for all information contained in the complete set of Contract Documents.
All Prime Contract Bidders who have paid the aforesaid deposit for an entire set of Bidding and Contract Documents; have submitted a bid with required bid security; and return such sets to the Architect in GOOD CONDITION within thirty (30) calendar days after the award of contract or rejection of bids, shall receive a refund of the full amount of such deposit for one (1) set. Partial reimbursement, in an amount equal to full deposit less actual cost of Bidding and Contract Document reproduction for one (1) set, shall be made for the return of all other sets in GOOD CONDITIONS within thirty (30) calendar days after award of contracts or rejection of bids.
As bid security, each Bid shall be accompanied by a certified check or Bid Bond made payable to Owner, in accordance with the amounts and terms described in the INSTRUCTIONS TO BIDDERS.
The Owner requires that all bids shall comply with the bidding requirements specified in the INSTRUCTION TO BIDDERS. The Owner may, at its discretion, waive informalities in bids, but is not obligated to do so, nor does it represent that it will do so. The Owner also reserves the right to reject any and all bids. Under no circumstances will the Owner waive any informality that, by such waiver, would give one Bidder a substantial advantage or benefit not enjoyed by all other Bidders. No Bidder may withdraw his Bid before forty-five (45) days after the actual date of the opening thereof, unless a mistake to error is claimed by the Bidder in accordance with INSTRUCTIONS TO BIDDERS.

The offer to be made in accordance with this Bid Solicitation shall include a bid on the following:
A bid price per ton, to provide equipment and labor for hauling solid waste and disposing solid waste at the Contractor’s Solid Waste Disposal Site. The term of this Agreement shall be two (2) years commencing on July 1, 2011, with the potential for three (3) additional option years (see Section 18.0, p. 21 of the specifications). Notwithstanding contractual or other legal reasons for terminating this Agreement, this Agreement will be guaranteed for a two (2) year term, through June 30, 2013.
Bids must be made in writing on the forms furnished and shall be accompanied by a Bid Guaranty in the Form of certified check, money order, bank draft or standard form letter of credit made payable to Town of Southold, or bid bond, in the sum of one hundred thousand dollars ($100,000.00) wherein the named obligee shall be the Town of Southold.
The successful Bidder shall be required to furnish a performance Bond and insurance in accordance with the instructions in the Bid Solicitation.
The bid price shall not include any tax, Federal, state, or local, from which the Town of Southold is exempt.
A Bidder may not withdraw his bid within forty-five (45) days after the opening of the bids, but may withdraw his Bid at any time prior to the scheduled time for the opening of bids.
The Town reserves the right to reject any or all bids and to waive informalities, should this action be in the best interest of the Town of Southold.
