UPDATE: New management identified at San Simeon, more questions remain
Greenport’s San Simeon by the Sound Center for Nursing and Rehabilitation is under new management and has an entirely new board of directors, The Suffolk Times has confirmed.
The nursing home is now operated by Advanced Health Inc., which is part of a larger entity called TL Management, according to a former San Simeon staff member who worked there for a decade. TL Management owns more than 100 skilled nursing care facilities in the United States.
After receiving an anonymous tip about suspected leadership changes, The Suffolk Times made multiple attempts to reach out to San Simeon by the Sound and eventually made contact with the facility’s new administrator, Subrina Charles, who confirmed she has filled the position. When a reporter asked to speak with San Simeon’s chief financial officer, Amy Giangregorio, a receptionist said she no longer works at the facility.
As of this week, the staff bios and board of director information pages on San Simeon’s website have been taken down. It is unclear what other administrative staff members have left San Simeon.
Ms. Charles declined to comment on the changes and told The Suffolk Times the management’s main office would, “be in touch.”
San Simeon’s former board chairperson, Leueen Miller, said in a recent email that she is “no longer involved” with the organization and that the other board members — Susan Flatley, the Rev. Garret Johnson, Daniel Ross, Richard Vessa and Arthur Williams — have also resigned and been replaced.
Mr. Ross, who served as board president at one time, also confirmed by phone he is no longer a member of the board of directors. San Simeon will continue to function as a not-for-profit, Ms. Miller wrote, and its mission has not changed.

Established in Greenport the 1970s, San Simeon is a 120-bed skilled nursing and rehabilitation facility that occupies a site on County Road 48 that is owned by St. Peter’s Lutheran Church.
In December 2023, the nursing home was reportedly facing a permanent shutdown due to financial troubles. After dealing for several years with the deadly COVID-19 pandemic, as well as staffing shortages and increasing costs, Ms. Miller said at the time that if the facility did not find a partner to help sustain its operations, it would be forced to close as of February 2024.
The nursing home leases roughly four acres of church property. Ms. Miller previously stated that any decision regarding its future must involve the San Simeon board, as well as the St. Peter’s board and congregation.
Since then, San Simeon by the Sound has continued its operations. In the months since the initial interview with Ms. Miller in December, several people have reached out to The Suffolk Times to share their experiences with the nursing home.
Many expressed concern for loved ones in the Greenport facility, citing issues such as inadequate care due to lack of staffing, alleged mistreatment from aides, neglect and poor sanitary conditions.
The New York State Department of Health issued 33 citations to San Simeon between Aug. 1, 2020 and July 31, 2024, exceeding the statewide average of 25 citations, according to the state website. In this time period, the health department recorded 49 total complaints regarding San Simeon.
